Education Plus! Mini-Grant Report Template

Submitting this project report is required of all 2011-2012 Mini-Grant recipients. Recipients who do not submit this project report will be considered ineligible to apply for a 2012- 2013 Education Plus! Mini-Grant.

Reports are due upon project completion or by August 31, 2012. Reports may be submitted via FAX, regular mail, in person, or by email. Please include a signed copy of this Report Form when submitting project information or attachments that cannot be sent electronically. Solano Community Foundation has the right to disseminate information about your project or materials developed.
